A compilation of all articles on the Committee of inquiry into the MRT disruptions that happened on Dec 15 and 17 last year which affected more than 200,000 commuters.

Led by Judge Tan Siong Thye, the Committee of Inquiry (COI) will also make recommendations and consider suggestions of SMRT, the Land Transport Authority (LTA) to improve train operations, prevent the recurrence of such disruptions and enhance the management of such incidents.

After the inquiry, the COI will prepare a report with its findings and recommendations. The report is to be submitted to the Ministry of Transport.

Week 5:

SMRT says rails safe despite cracks
Hairline cracks on the joints of the power-supplying third rail on the North-South and East-West lines do not pose safety concerns and do not affect the intergrity of the rails, said SMRT.
SMRT to replace third rail at some locations after cracks found
The move comes after a team of experts warned on Tuesday that parts of the rail could have hairline cracks and asked for immediate checks to be carried out.
MRT Inquiry hearings: Defective materials may explain breakdown
A defective fastener and two insulators used to support the third rail had been weakened due to manufacturing flaws.

Week 4:

SMRT to disseminate breakdown info better
Bedok MRT station will be the testbed for a pilot trial to be launched this month, to install new LCD screens that provide information on train-service disruptions and alternative bus services.
SMRT to implement measures to mitigate rail disruptions
Train operator SMRT will has said it will implement measures to help commuters in the event of service disruptions.
LTA not told of rail claw issues before Dec
The Land Transport Authority (LTA) was unaware of incidences of dislodged rail claws before last December's two major train disruptions.
COI reveals lapses in maintenance of trains
We are now at the halfway mark of the hearing into the two major train disruptions last December.
